A HIPAA Authorization allows you to authorize specific individuals to receive your protected health information (PHI) from healthcare providers, hospitals, and insurers.

The Health Insurance Portability and Accountability Act of 1996, or HIPAA, and the laws of any state where this form is presented.
⚠ IMPORTANT INFORMATION BEFORE YOU BEGIN:
- WHO SHOULD USE THIS FORM: Any adult (age 18 or older) of sound mind who wishes to authorize disclosure of their protected health information (“PHI”) to a named recipient.
- WHAT THIS FORM DOES NOT DO: This is not a Medical Power of Attorney. It does not authorize your recipients to make healthcare decisions on your behalf.
- WHEN IT TAKES EFFECT: Immediately upon signing. This Authorization should be provided to recipients and healthcare providers.
- DURATION AND REVOCATION: This Authorization remains valid until a written revocation is provided to any recipient. Any revocation does not apply to disclosures already made in reliance on this form, or where this Authorization was required to obtain insurance coverage, and other law provides the insurer with the right to contest a claim under the policy itself.
- VOLUNTARY EXECUTION: Signing is voluntary. No health care entity may condition treatment, payment, enrollment, or benefit eligibility on my signing this Authorization, except as expressly permitted by law.
